For Doctors & Professionals

A Referral Pathway You Can Rely On

We work with psychiatrists, geriatricians, hospital discharge teams and GPs across Petaling Jaya and the Klang Valley, placing adults who need residential psychiatric or dementia care. Referrals are acknowledged the same working day.

Referral Process

Send a Summary

Diagnosis, current medication, risk profile and mobility, by email or WhatsApp.

No referral form needed — a short clinical summary in whatever format you already have is enough to start.

  • Diagnosis, current medication list, risk profile and mobility status
  • Email or WhatsApp to our referral line — discharge summaries and photos of notes are fine
  • Acknowledged the same working day
  • Direct line to our nurse manager, Monday to Saturday, 9am–6pm

Suitability Response Within 24 Hours

Our nurse manager confirms whether the case is within our capability.

You get a clear yes or no within 24 hours — and if it's no, we say so early rather than at the bedside.

  • Confirmation the case is within our clinical capability
  • Exclusions flagged immediately: acute suicidality, active substance withdrawal or restraint needs
  • Bed availability and an indicative fee included in the response
  • Where we can't help, we suggest a more suitable setting if we know one

Joint Assessment

At hospital bedside or at our home, with family present where possible.

A joint assessment at the hospital bedside with family present

We assess together, so clinical handover and family expectations are aligned before anyone commits.

  • At the hospital bedside, our home or the patient's home
  • Family present where possible — their consent and understanding matter as much as the clinical picture
  • Risk and capability review documented and shared with you
  • A draft care plan prepared with your input

Admission

We coordinate transport, medication handover and the initial care plan with your team.

Admission day is fully coordinated with your team — nothing is left to the family to relay.

  • Transport arranged and timed with the ward or clinic
  • Medication handover charted directly onto our MAR from your prescription
  • The initial care plan agreed with the referring team before arrival
  • Progress summaries to the referring clinician from week one

Clinical Information

Registered-nurse-led 24-hour nursing · visiting doctor overseeing residents' general health · visiting consultant psychiatrist · full medication administration record (MAR) charting · monthly multidisciplinary reviews · incident reporting shared with the referring clinician on request. We do not manage acute suicidality, active substance withdrawal or conditions requiring restraint.

Discharge Coordination

For step-down cases we agree discharge criteria at admission, provide progress summaries, and hand back to the referring team with a full transfer report.
