Inside Our Monthly Multidisciplinary Review

How nursing, psychiatry and family input come together in one care plan.

A nurse, psychiatrist and family member reviewing a care plan together

Once a month, every resident's care is put on the table — literally. Here is how the multidisciplinary review works and what comes out of it.

Who Is in the Room

The nurse manager chairs; the visiting consultant psychiatrist, the resident's key care staff, and — by input or in person — the family. For step-down residents, the referring clinician's notes and discharge criteria sit at the centre of the discussion.

What Gets Reviewed

  • The month's MAR: adherence, PRN frequency, side effects
  • The behaviour and incident log — patterns, not just events
  • Activity participation, sleep, appetite and weight
  • Progress against the care plan's goals, including rehabilitation targets

What Comes Out

An updated care plan signed off by the psychiatrist, a written summary to the family through their named coordinator, and — for referred residents — a progress note to the referring team.
