Medication Handover: Our MAR Charting Standard
The records we keep, how doses are double-checked, and what we report back.
Medication error is the most preventable harm in residential care, so this is the part of our operation we run with hospital-grade discipline. Here is what a referring clinician can expect.
At Handover
Your prescription or discharge summary is charted directly onto our medication administration record (MAR) at admission — drug, dose, route, timing and PRN criteria. Discrepancies between the script and what the family brings are resolved with you before the first dose, not after.
Day to Day
- Every administration is charted at the time it happens — no end-of-shift batch signing
- Dispensing is double-checked by two staff, so no dose depends on one pair of eyes
- All medication is held in locked, audited storage
- PRN use is recorded with indication and outcome, reviewable at the monthly psychiatric review
What We Report Back
On request, referring clinicians receive MAR extracts, missed-dose and refusal logs, PRN frequency, and observed side effects. Changes recommended by our visiting psychiatrist are communicated to you and to the family before they take effect, and full MAR records travel with the resident at every transfer of care.
