Step-Down Referral Criteria: What We Accept and Why

Our clinical capability, exclusions and the information that speeds up a referral.

A clinical criteria checklist being reviewed with a stethoscope beside it

We accept step-down referrals where a patient is stable enough for community residential care but not ready — or not able — to return home. This page summarises capability so you can triage before you call.

Within Our Capability

  • Psychiatric conditions requiring supervised daily living and medication management
  • Dementia with behavioural symptoms: wandering, agitation, sundowning
  • Post-admission step-down with agreed discharge criteria
  • Long-term residents needing structure, observation and MAR-charted medication

Exclusions

We do not manage acute suicidality, active substance withdrawal, or presentations requiring restraint. We flag these at the 24-hour suitability response — never at the bedside.

What Speeds a Referral Up

  • Diagnosis and current medication list (a photo of the chart is fine)
  • Risk history: harm to self or others, absconding, falls
  • Mobility and transfer status
  • Family contact and their understanding of the plan

Send summaries by email or WhatsApp to our referral line — acknowledged the same working day, suitability confirmed within 24 hours.
